REGISTRATION FORMS

Special note for teams registering for our first season or two in operation:  Until things get firmly established, there is a chance there may not be enough teams sign up for particular day to warrant forming a league.  As such, please make a note on your registration/roster form if there are any other days on which your team would be able to play (or if there are certain days that your team CANNOT play).  This will help us in our effort to ensure that every team who wants to play has the best possible chance to secure a spot in a league, even if it is not your absolute first choice of day.  We appreciate your patience and understanding as we move through this stage of initial growth.
Remember, all weeknight leagues kickoff times are between 6:30 and 10:30pm.  Weekend leagues are offered throughout the day.

* For forms that require signatures, we cannot accept via fax.  Completed forms may be submitted via email (i.e., print, fill out, and scan) or by mailing/hand-delivering to our office.
